Vendor Applications

We appreciate your company’s interest in providing goods and/or services to the City of Middletown. In an effort to update our vendor database we are requesting all new and existing vendors to complete a vendor application (PDF) which now includes a general listing of goods and services. Please complete this listing by checking any commodity item(s) that your company may be able to provide to the city and return the completed form to the Purchasing Department.

In addition, the City of Middletown’s bidding threshold for goods and services is over $24,999. All goods and services in excess of this amount may be formally bid.
