CONTRACTORS - WE WILL NOT REVIEW YOUR APPLICATION UNTIL WE RECIEVE A COPY OF YOUR CONTRACTORS LICENSE ALONG WITH A CERTIFICATE OF LIABILITY INSURANCE W/ WORKERS COMP & THE CHECK FOR YOUR PERMIT FEE.
HOMEOWNERS - IF YOU ARE APPLYING FOR THE PERMIT AND DOING THE WORK YOURSELF YOU WILL NEED TO SUBMIT A NOTARIZED 7B FORM (SEE BELOW) ALONG WITH YOUR CHECK FOR YOUR PERMIT FEE. IF YOU HIRED A CONSTRACTOR BUT ARE APPLYING FOR THE PERMIT FOR THEM, A 7A FORM (SEE BELOW) IS REQUIRED ALONG WITH THE CHECK FOR YOUR PERMIT FEE.
**IF YOUR PROPOSED WORK REQUIRES PLANS YOU WILL NEED TO PROVIDE FULL SIZE DRAWINGS TO US ALONG WITH OTHER REQUIRED PAPERWORK AND YOUR PAYMENT BEFORE WE REVIEW ANY INFORMATION**
